Tibialis posterior is a tendon in the back of your ankle that connects the calf muscles to the arch in your foot. One key function of the tibialis posterior tendon is to maintain the healthy shape of the arch.

PTTD occurs when weaknesses in the calf, foot or other parts of your lower limb overburden the tibialis posterior tendon, which gets inflamed or damaged from repetitive stress.

The tendon becomes less effective at maintaining the shock-absorbing spring of the arch, leading to foot or ankle pain, overpronation of the foot, knee problems, and other issues up and down the kinetic chain.

The tibialis posterior tendon works with many muscles in your foot, calf and up throughout the kinetic chain to function. That's why you won't work just on the tibialis posterior in the PTTD Guided Recovery Program.

Rather, you'll be building strength and stability in the entire chain, so that you can give the injured tendon more support, help rebuild the healthy arch, and prevent future flare-ups.

Some physical therapists and many online resources still have you do exercises that aggravate your pain. Others overemphasize stretches for PTTD. However, these approaches have been proven ineffective.

Eccentric exercises - the type of exercises that load your muscles as they lengthen - are much more effective at treating PTTD and building strength. They are also less likely to cause further pain.

That's why you'll primarily focus on eccentric exercises in the Guided Recovery Program, with other types mixed in for best long-term results.

Because PTTD is a repetitive stress injury that's easily aggravated, it's important to pace yourself. Good PTs assess each patient's progress and customize the next step.
